Cabinetry, then labour, then worktops. Appliances and tile feel like the big decisions because they are the ones you shop for, but they are rarely what moves the total.
The other thing worth settling early is whether you are moving plumbing or electrics. Keeping the sink and the run of the units where they are can be the difference between a few weeks of work and a project.
Taking a wall out, as in the kitchen above, is usually the change people are happiest with afterwards. It is also the one that needs thinking about first, because it is structural.
